Dear Patients,
IF YOU ARE ASKED BY GP, NURSE OR HOSPITAL TO PROVIDE A SPECIMEN OF FAECES, URINE, SPUTUM ETC. IT IS VERY IMPORTANT THAT YOU FOLLOW THE GUIDELINES BELOW:
- The sample collection box is situated on wall beside back door of the practice.
- The sample box will only be open between 8am and 12 mid-day.
- Samples will not be accepted after 12 mid-day.
- All samples need to be dated and timed on the form (if provided) and sample bottle.
- Samples will only be accepted in specimen containers that have been provided by the GP/Practice Nurse/Hospital or Chemist.
- Please ensure form and outside of specimen container has not been contaminated with sample.
- URINE Samples :
If you think you have a urinary infection you must contact reception and complete a symptom form first, samples
will not be tested unless this form is completed.
